The Foundation is now accepting applications online only.

To apply for funding you must complete this application form and email (in PDF form if possible) any supporting documents to grants@ccmfoundation.org. Please note that this email address is for document submissions only. Questions may be submitted via US Postal Service at P.O. Box 282, Grandville, Michigan 49468-0282.

The Foundation only considers applications for grants to organizations focusing on the Greater Grand Rapids Michigan Area in the United States of America.

Applications are evaluated quarterly and are due January 3, April 3, July 3 or October 3 in order to be eligible for consideration for the next quarter.
